Do I Have to Increase My Dosage If I’m Happy With My Progress?

If we recommend increasing your dosage, you might wonder whether it’s essential, especially if you feel happy with your progress.

If your hormone levels need to be increased to stay within the safe, healthy range, then we will advise and recommend an increase.

If your levels are already within the normal range, we may still suggest a higher dose to help you achieve your goals but if you prefer to continue at your current pace, we will issue your Treatment Recommendation at the same dose.


Why dosage recommendations matter

Hormones are vital for your whole body. They support:

  • Brain health – mood, focus, emotional balance

  • Bone strength – protection against thinning and fractures

  • Body function – energy, sexual health, and overall wellbeing

It’s important that your hormone levels stay within the physiological ‘normal’ range, not too high, and not too low.


Taking things at your own pace

Everyone’s transition looks different. You may choose to:

  • Stay at the lower end of the range if you want changes to happen gradually.

  • Move toward the higher end of the range if you want faster or more noticeable changes.

  • Keep a fair balance of both oestrogen and testosterone if you are non-binary.


Our algorithms make recommendations based on:

  • Your personal goals

  • How quickly you want to achieve your transition

  • Which, if any, blocker you are taking

These recommendations are a guide, not a rule.


Your body, your transition

You are always in control. You don’t have to accept every recommendation. What matters most is that your oestrogen or testosterone levels remain safely within the normal range.

  • Too little can leave your body under-supported.

  • Too much can cause unwanted side effects.

As long as your levels are balanced, you can decide whether to increase your dosage now, later, or not at all. If you have requested to keep the same dose, we may recommend an increase but whether you do that or not is up to you.

If there is a need to increase then we will suggest that too.


What is ‘Normal’?

For transgender men (FTM):

  • Target testosterone: 12–30 nmol/L

For transgender women (MTF):

  • Target testosterone: below 5 nmol/L

  • Estradiol should remain between 176 and 936 pmol/L


For non-binary people:

  • A mixture of both hormones is fine, depending on your goals, but it’s still important to have enough hormones on board to support your brain, bones, and body.
