Feminisation Timeline

Hormone therapy takes time, but when can you expect to see changes? Although everyone’s journey is different, there are general timeframes you can expect to see changes in yourself. 


You can use this guide to see what you can expect from hormone therapy. Bear in mind that your experience might not perfectly line up with these estimates.

1 to 4 Weeks

  • Skin feels softer and appears smoother.

  • Skin oil production decreases, with some people noticing less acne or more hydrated feeling skin.

5 to 8 Weeks

  • More skin softening.

  • Body hair may grow slower or become finer.

  • Body chemistry continues to adjust to support later changes.

  • Greater emotional clarity and increased confidence.

9 to 12 Weeks

  • Body shape starts changing as fat is redistributed, softening the hips and thighs.

  • Beginning of reduced muscle mass.

  • Emotions feel more balanced, enhancing vulnerability and joy.

  • Increased connection with oneself is common.

4 to 6 Months 

  • Early breast development and tenderness.

  • Facial features soften, giving a more rounded appearance.

  • Fat redistribution becomes more visible and muscle mass decreases slightly for a softer, curvier appearance.

  • Many people feel a sense of peace and relief with the changes. 

  • Greater emotional resilience and self-acceptance are common.

6 to 9 Months

  • Breasts grow fuller.

  • Waistline softens.

  • Reduced body and facial hair growth become noticeable.

  • Hair texture may feel finer or softer.

  • An energy boost as circulatory and hormonal balance improves.

  • Increased confidence and comfort in gender expression.

  • Greater empathy and openness help strengthen personal connections.

9 to 12 Months

  • Breasts become rounded and defined

  • Fat redistribution continues, particularly around the hips and thighs.

  • Facial features take on a more gentle shape.

  • Oestrogen supports cardiovascular health, improving overall wellness and energy. 

  • Feelings of joy, self-assurance and acceptance are common. 

12 to 18 Months

  • Breast size stabilises.

  • Body curves are now characteristically feminine. 

  • Hormonal balance and metabolic rate adjust, supporting sustained physical changes.

18+ Months

  • All physical changes settle into a steady state.

  • Breasts are fully developed and fat redistribution is stable.

  • Strong fulfilment, confidence and identity.

  • Many say they feel at home in their bodies, and feel renewed in purpose.

  • Long-term benefits of oestrogen (enhanced bone density, heart health) support vitality.



Important things to remember


  • Patience is key. These changes are gradual, and they aren’t guaranteed to happen exactly according to the timing in this table. Everyone’s transformation is unique to them.

  • Celebrate. When you notice a change – even small – it’s a step towards your goal. Every milestone is worth noticing and appreciating.
  • Build a support network. In a period of change like this, having people you can speak to who understand you and you can trust is a huge help. Friends, family, community members, support groups: any of these can be incredibly helpful to you.
