How do I request my records for a new provider?
To get a copy of your Care Summary, latest Treatment Recommendation, or your assessment report, simply email us at data@gendergp.com.
You should already have these documents in your inbox from when we first sent them, but we’re happy to provide your Care Summary which is especially helpful for your new provider.
We wish you all the best with your future provider, and please remember, you’re always welcome to return to us for full services or any additional support that your new provider may not offer.
