There will be sections about you, your demographic information, and your medical history (to evaluate your health). Then, there will be sections asking you for your preferences and choices.
After you submit your form, you'll need to schedule an Information Gathering Session (IGS) with a specialist. The purpose of the session is to confirm that you understand the choices you are making and that you are making informed decisions.
